Even if you're paying by PayPal we still need it to keep your details up to date so we can keep in touch with you.
 

The form can be returned in one of 4 ways:

• fill & submit using "submit by email" button on the form - only works if you have a dedicated email client (e.g. Outlook)
• fill, save it to your hard drive and attach to email manually - use this method for web based mail (e.g. hotmail, gmail, yahoo, etc.)
• fill, print, scan and email it manually - more tedious but most reliable
• print it to fill in by hand and return by snail-mail - for those without email access
